  1. That I am a permanent citizen and national of Bangladesh by birth.

  2. That my daughter, Tasnim Rahman Orni, passed the SSC Examination in the year 2015 from Dhaka Board.

  3. That in the certificate of SSC Examination, her name was incorrectly stated as Tasnia Rahman Orni instead of Tasnim Rahman Orni.

  4. That her actual and correct name is Tasnim Rahman Orni. This affidavit is submitted as documentary evidence to support the correction of her name and shall be presented to the concerned authorities when required.

  5. That I declare that all the information provided above is true and correct to the best of my knowledge.

The statements made above are true to the best of my knowledge and belief, and in witness thereof, I sign this affidavit.
