Affidavit Guide for Conversion to Islam in Bangladesh

This affidavit is prepared to formally declare the conversion of an individual from one religion to another. Such legal documents are essential for updating official records, bank accounts, and identification papers. A notarized affidavit ensures that the change of religion is recognized legally in Bangladesh and prevents any future disputes regarding identity or personal status.

I, Rafael van Dijk, S/O. Johan van Dijk, of Rotterdam Street 14, 3011 AB, Netherlands, presently residing at 34/A Greenfield, Dhanmondi, Dhaka-1209, Bangladesh, Date of Birth 5th March, 1955 AD, Nationality: Dutch by birth, Religion: Christian, do hereby solemnly affirm and declare as follows:

  1. I am a citizen of the Netherlands. My Passport No. is NL 56K9P3.

  2. From my childhood, I associated with my Muslim neighbors and gradually learned about their faith. I studied the Bengali and English versions of the Holy Quran and the life story of Prophet Hazrat Muhammad (SM). Over time, I became deeply attracted to Islam and decided to embrace it.

  3. Accordingly, on the 10th day of May, 2012, I embraced Islam by reciting the Kalima “La Ilaha Illallahu Muhammadur Rasulullah (SM)” in the presence of witnesses under the guidance of the Imam of the local mosque at my present address. I adopted the name Rafiqul Islam.

  4. Henceforth, my name shall be Rafiqul Islam, and I request to be called, addressed, and recorded by this name in all official and personal records.

  5. I declare that I embraced Islam voluntarily, without any undue influence, pressure, or intimidation from any person or entity.

  6. From today, all my previous names and identifiers used in banks, official transactions, documents, or elsewhere shall be replaced with my new Muslim name, Rafiqul Islam.

  7. That I also intend to update my personal records in government offices, financial institutions, and all other relevant authorities to reflect this change.
